Moms In Motion has an immediate opening for an SF Courier Case Manager 

The SF Courier works closely with the SF Data Clerk to support the families we serve. The SF Courier visits the client’s home, reviewing documentation, obtaining signatures, and uploading for processing to ensure no lapse in client services.

This is a salaried (plus mileage), non-exempt, full-time position (40 hours) with benefits.

Requirements:
  • Must be available to work a flexible schedule based on client needs.
  • Must have a reliable vehicle to go to client homes
  • Must have a portable device with internet connection and signature capture capability.
  • Must have a minimum of an Associate's Degree in Human Services or similar field with 2 years relevant experience; or a Bachelor's Degree in any field and 3 years relevant experience
  • Must be highly organized, proficient in scheduling, dependable and an expert on documentation
  • Requires a documented combination of skills/relevant work experience. Person-Centered Thinking/Planning training is a plus. 
  • Knowledge of Medicaid waivers is a plus.
